Dyvine Medical & Wellness Peptide Wellness Pathway Booking Policy This policy outlines booking expectations for clients enrolling in peptide wellness pathways at Dyvine Medical & Wellness (DMW). These pathways include wellness coordination, education, and ongoing support services. DMW does not diagnose, prescribe, dispense, or ship medications. All medical decisions are made solely by the licensed medical provider. Scheduling & Payment All appointments must be scheduled in advance through the DMW booking system. A valid payment method is required to reserve your appointment. Enrollment and booking fees cover administrative coordination, intake review, education, and wellness support provided by DMW. Payments do not represent the purchase of medication. Provider Evaluation & Eligibility Participation in a peptide wellness pathway does not guarantee medication approval. Clients must complete intake forms, health history documentation, and any required labs prior to provider review. Medication eligibility, dosing, and treatment decisions are determined exclusively by the licensed medical provider. Cancellations & Rescheduling Appointments may be rescheduled with at least 24 hours notice. Cancellations made with less than 24 hours notice, missed appointments, or late arrivals may result in forfeiture of the booking fee and require a new appointment payment. Medication Fulfillment If treatment is deemed appropriate, prescriptions are issued by the licensed provider and fulfilled directly by a partnering pharmacy. Medications are shipped from the pharmacy to the client. DMW does not handle medication storage, dispensing, or shipping. Client Responsibilities Clients agree to provide accurate information, complete required documentation prior to appointments, follow provider and pharmacy instructions, and maintain communication regarding scheduling and support needs. Results & Expectations Individual experiences vary. Enrollment in a peptide wellness pathway does not guarantee specific results.
