Dyvine Medical & Wellness (DMW)

By accessing services, booking appointments, or enrolling in programs offered by Dyvine Medical & Wellness (“DMW,” “we,” “our,” or “us”), you agree to the following terms and conditions.

1. Scope of Services

Dyvine Medical & Wellness provides wellness coordination, specimen collection, education, administrative support, and structured wellness pathway programs. DMW does not diagnose, treat, prescribe, or provide medical care.

Participation in any service or program does not guarantee specific outcomes. Results vary based on individual participation, lifestyle factors, and provider recommendations.

2. Independent Medical Providers

All medical evaluations, prescriptions, and clinical decisions are performed solely by licensed healthcare providers operating independently of DMW.

Clients are responsible for communicating directly with their provider regarding eligibility, treatment questions, side effects, and medical concerns. If prescribed, medications or wellness supplies are dispensed and shipped directly by licensed partner pharmacies. DMW does not store, dispense, or handle medications.

3. Appointments & Scheduling

Appointments must be attended as scheduled or rescheduled with reasonable notice. Late arrivals may result in shortened sessions or rescheduling. Missed appointments may be forfeited depending on service type.

Virtual appointments require clients to join on time with a stable internet connection.

4. Payment & Billing

Fees cover coordination, education, administrative services, and pathway participation as outlined in each offering. Payment plans reflect participation in structured services and are not contingent upon provider approval or individual outcomes.

All payments are processed through secure third-party payment processors.

5. Refund & Cancellation

Program fees and coordination services are non-refundable once enrollment, consultation scheduling, or administrative coordination has begun.

Monthly pathways may be canceled prior to the next billing cycle when applicable. Multi-month structured programs cannot be canceled mid-cycle once services have started.

6. Client Responsibilities

Clients agree to:

• provide accurate intake information
• communicate respectfully with staff and providers
• attend or reschedule appointments when necessary
• follow provider guidance for any prescribed wellness supplies
• participate actively in pathway support and communication

7. Educational Disclaimer

Information provided by DMW is educational and supportive in nature and is not a substitute for professional medical advice. Clients should consult licensed healthcare providers for medical concerns.

8. Limitation of Liability

DMW is not responsible for provider decisions, pharmacy fulfillment, shipping timelines, or individual responses to prescribed therapies. Participation in services is voluntary and at the client’s discretion.

9. Privacy & Data Use

Client information is collected and used only as necessary to provide services and improve client experience. Personal information is handled in accordance with our Privacy Policy.

10. Policy Changes

DMW reserves the right to update these terms at any time. Continued participation in services indicates acceptance of any revisions.

11. Agreement

By booking services or enrolling in programs, clients confirm they have read, understood, and agreed to these Terms & Conditions.
