Cancellation Policy

At Dyvine Medical & Wellness (DMW), our Wellness Pathways are structured programs that require dedicated scheduling, coordination, and administrative preparation. To protect program integrity and ensure fair access for all clients, the following cancellation policy applies.
Consultation & Appointment Cancellations
Appointments must be canceled or rescheduled at least 24 hours in advance.
Cancellations made with less than 24 hours’ notice may result in the appointment being forfeited or a rescheduling fee.
Repeated missed appointments may impact program effectiveness and availability of future scheduling.

Pathway Program Commitment
Reset, Ignite, Elevate, and Recovery Pathways are structured multi-month wellness programs designed to support consistency and long-term progress.
Once enrollment, intake review, consultation scheduling, or coordination has begun, program participation cannot be canceled mid-cycle, and remaining program fees are not refundable.
Participants may continue using available program support and check-ins for the remainder of their enrollment period.

Essentials Pathway (Monthly Option)
The Essentials Pathway operates on a monthly structure. Clients may cancel future billing after completing the first month by providing notice prior to the next billing date. Previously processed payments are non-refundable once services have begun.

Provider & Pharmacy Services
Medical evaluations, prescriptions, and pharmacy fulfillment are managed by independent licensed providers and partner pharmacies. Cancellation of pathway participation does not cancel provider services, medical evaluations, or pharmacy orders already in progress.
Clients must communicate directly with their provider regarding treatment-related questions or adjustments.

Emergency & Special Circumstances
DMW understands that unexpected situations occur. Cancellation flexibility may be considered on a case-by-case basis at DMW’s discretion; however, approval is not guaranteed.
