HSA / FSA & Wellness Benefits

Many of our services may be eligible for payment or reimbursement through HSA, FSA, HRA, and employer wellness benefit programs (including platforms such as Forma and other lifestyle benefit providers).

DMW accepts standard debit and credit card payments, including HSA/FSA cards. Clients may also request itemized receipts for submission to their benefits provider when reimbursement is required.

​

Eligible services commonly include preventive wellness support, specimen collection, biometric screenings, and health coaching services. Coverage and reimbursement are determined by your individual benefits plan.

If you plan to use a health savings or wellness benefit, we recommend confirming eligibility directly with your provider prior to your appointment.
